Shared Horizons' works diligently to provide a quality service without jeopardizing our beneficiaries' assets and benefits.  We are committed to enhancing the quality of life for each beneficiary through the creation of an individualized plan and budget.  Although many of our beneficiaries live independently, others have an extended support system through provider services.  We depend on the provider's extended knowledge of the beneficiary to assist with creating and implementing a quality of life plan that meets the needs and desires of our beneficiaries.

Once the Quality of Life Plan is created, the provider, if designated by the Grantor, may serve as a representative on behalf of the beneficiary.  This representative serves as the liaison to the Trust.  They can make requests, obtain statements, and act a personal shoppers for the beneficiary.
